जनवरी के आख़िरी में कोरोना वायरस संक्रमण का पहला मामला भारत में आया था और तब से अब तक डेढ़ लाख से ज़्यादा मामले संक्रमण के सामने आ चुके हैं। अब तक चार हज़ार लोगों की संक्रमण की वजह से मौत भी हो चुकी है।
22 मई तक भारत में कोरोना टेस्ट का पॉजिटिव दर चार फ़ीसदी था और संक्रमितों में मृत्यु दर करीब तीन फ़ीसदी तक है। 13 दिनों में कोरोना के मामले यहाँ दोगुने हो जा रहे हैं। वहीं संक्रमितों के ठीक होने का दर 40 फ़ीसदी है। यह बुरी तरह से प्रभावित कई देशों की तुलना में काफ़ी कम है। दुनिया के तमाम दूसरे देशों की तरह यहाँ भी संक्रमण के हॉटस्पॉट और क्लस्टर हैं।
सरकारी आँकड़ों के मुताबिक अस्सी फ़ीसदी से ज़्यादा मामले पांच राज्यों महाराष्ट्र, तमिलनाडु, दिल्ली, गुजरात और मध्य प्रदेश में हैं और साठ से ज़्यादा संक्रमण के मामले पांच शहरों से है जिनमें मुंबई, दिल्ली और अहमदाबाद जैसे शहर शामिल हैं।
जिन लोगों की संक्रमण से मृत्यु हुई है उनमें से आधे से ज़्यादा साठ या उससे अधिक उम्र के लोग हैं। यह उन अंतरराष्ट्रीय आँकड़ों की तस्दीक करते हैं जिनके मुताबिक बुजुर्गों में संक्रमण का जोख़िम ज़्यादा है।
क्या कहते हैं सरकारी आँकड़े?
दो महीने से ज़्यादा के लॉकडाउन की वजह से सरकारी आँकड़े बताते हैं कि 37000 से लेकर 78000 लोगों की जान बचाई जा सकी है।
हार्वड डाटा साइंस में छपा एक पेपर भी इन आकड़ों की पुष्टि करता नज़र आता है। इस पेपर के मुताबिक़ आठ हफ़्तों के लॉकडाउन में दो लाख संक्रमण के मामलों को रोका जा सकता है और तीन फ़ीसदी तक मृत्यु दर कम की जा सकती है और करीब साठ हज़ार लोगों की जान बचाई जा सकती है।
कोविड-19 को लेकर बनाए गए आपातकालीन प्रबंधन योजना के मुखिया वीके पॉल कहते हैं, "संक्रमण कुछ निश्चित इलाक़ों तक ही सीमित हो चुका है। इसने हमें लॉकडाउन के अंदर कुछ क्षेत्रों में ढील देने का हौसला दिया है। यह अब तक मुख्य तौर पर शहरी क्षेत्र की ही बीमारी बनी हुई है।"
कुल संक्रमण के मामलों के हिसाब से भारत दुनिया के दस शीर्ष देशों में शामिल हो चुका है और नए संक्रमण के मामलों में यह दुनिया के पांच देशों में एक है।
संक्रमण के मामलों में तेज़ी आई है। 25 मार्च को जब पहले चरण का लॉकडाउन शुरू हुआ था तब उस वक़्त सिर्फ़ 536 मामले थे।
संक्रमण के तेज़ी से बढ़ते हुए मामलों को देखते हुए अप्रैल की तुलना में टेस्टिंग की दर दोगुनी हो गई है लेकिन संक्रमण के मामले भी इस दौरान चार गुणा हो चुके हैं।
क्यों बढ़ रहा है संक्रमण?
महामारी विशेषज्ञों का कहना है कि संक्रमण के मामलों में इजाफ़े की रिपोर्ट संभवत: इसलिए बढ़ती हुई दिख रही है क्योंकि पहले की तुलना में टेस्टिंग अब ज़्यादा हो रही है। पिछले हफ़्ते एक लाख सैंपल पूरे देश में टेस्ट के लिए गए हैं।
टेस्ट करवाए जाने वालों के दायरे में अब उन लोगों के संपर्क में आने वाले लोगों को भी शामिल कर लिया गया है जिनमें कोई लक्षण नहीं (एसिम्प्टोमैटिक) दिखाई पड़ रहे हैं। इसके बावजूद भारत की जगह दुनिया में सबसे कम टेस्ट दर वाले देशों में बनी हुई है। यहाँ प्रति दस लाख आबादी पर सिर्फ़ 2,198 टेस्ट ही हो रहे हैं।
मार्च के अंतर में शुरू हुए लॉकडाउन की वजह से नौकरी और काम धंधा गंवा चुके लाखों प्रवासी मज़दूरों को शहर छोड़कर गांव अपने घरों को लौटना पड़ रहा है। शुरू में तो ये प्रवासी मज़दूर पैदल ही अपने घरों को लौट चले थे लेकिन इसके बाद उन्होंने ट्रेन चलाए जाने के बाद ट्रेन से जाना शुरू किया। अब तक चालीस लाख मज़दूर रेल से पिछले तीन हफ़्तों में दर्जन भर राज्यों में अपने गांव लौट चुके हैं।
अब इस बात के प्रमाण मिलने लगे हैं कि शहरों से गांव में संक्रमण फैलना शुरू हो चुका है। इस महीने की शुरुआत में लॉकडाउन में मिली छूट के बाद इस बात का डर बढ़ गया है कि शहरों में भी अब और संक्रमण बढ़ेंगे।
संक्रमण के बढ़ते मामलों के बावजूद मृत्यु दर में कमी इस बात की ओर इशारा करती है कि युवाओं में संक्रमण का स्तर या तो कम है और जो है भी उनमें बड़ी संख्या में एसिम्प्टोमैटिक मरीज हैं। नीति आयोग के सीईओ अमिताभ कांत कहते हैं कि हमारा ध्यान मौत के मामलों को कम करने और ज्यादा से ज़्यादा लोगों को ठीक करने पर होना चाहिए।

हालात गंभीर होने की चेतावनी
लेकिन संक्रमण का दर बढ़ता ही जा रहा है। एक शीर्ष वायरोलॉजिस्ट का कहना है कि, "आने वाले कुछ हफ़्तों में हालात गंभीर होने जा रहे हैं।"
दिल्ली और मुंबई के डॉक्टरों का कहना है कि कोरोना संक्रमण के मामले में इजाफ़े को देखते हुए अस्पताल में बिस्तर और दूसरे संसाधनों की मौजूदगी चिंता का सबब है।
जुलाई में संक्रमण के मामलों में तेज़ी आने की उम्मीद है। इसके बाद अस्पताल में बिस्तर और पर्याप्त सुविधाओं के ना होने की वजह से कई लोगों को मरने से नहीं रोका जा सकेगा।
इंदौर में कोरोना वार्ड में ड्यूटी कर रहे रवि दोसी कहते हैं, "असल चिंता की बात यही है। एक गंभीर रूप से संक्रमित व्यक्ति को ऑक्सीजन लगे बिस्तर, वेंटिलेटर, डॉक्टर और नर्स सब की ज़रूरत पड़ती है। सब कुछ फिर दबाव में होगा।"
50 बिस्तरों का उनका आईसीयू पहले से ही कोराना संक्रमितों से भरा हुआ है।
लॉकडाउन में छूट मिलने के साथ ही डॉक्टरों के हाथ-पांव फुलने लगे हैं। डॉक्टर दोसी कहते हैं, "यह काफी डराने वाला है क्योंकि कई लोगों ने दफ़्तर जाना शुरू कर दिया है। लेकिन अब भी बहुत ख़तरा है।"
वो कहते हैं, "कोई एक दफ्तर में सहकर्मी छींकता है तो 10-15 उसके साथ काम करने वाले विचलित हो जाते हैं और वो भागे-भागे अस्पताल आते हैं और टेस्ट की मांग करते हैं। इससे दबाव की स्थिति पैदा हो रही है।"
इस महामारी को लेकर पर्याप्त डेटा की कमी भी एक ऐसी वजह जिससे क्नफ्यूज़न की स्थिति पैदा हो रही है और रणनीति बनाने में मुश्किल पैदा हो रही है।
कई विशेषज्ञों का कहना है कि पूरे भारत के लिए लॉकडाउन लगाने और उसे हटाने को लेकर एक जैसी रणनीति यहाँ काम नहीं करने वाली है। यहाँ पर हर राज्यों की स्थिति के हिसाब से रणनीति बनाने की ज़रूरत होगी क्योंकि हर राज्य में अलग-अलग समय पर संक्रमण में इजाफ़ा देखा जा रहा है। अब महाराष्ट्र का ही उदाहरण देख लीजिए। वहाँ प्रति सौ टेस्ट पर संक्रमण का दर है वो राष्ट्रीय स्तर के औसत से तीन गुना ज़्यादा है।
डॉक्टरों की हालत ख़राब
फिर उन तीन हज़ार मामलों का क्या जो किसी भी राज्य ने अपने यहाँ के आँकड़ों में शामिल नहीं किया है क्योंकि ये लोग वहाँ संक्रमित पाए गए जहाँ वो रहते नहीं हैं। इनमें से कितने ठीक हुए, कितने मरे कुछ पता नहीं। अब आप इसे संदर्भ में देखिए कि भारत में नौ राज्यों में तीन हज़ार से ज़्यादा मामले हैं। यह भी स्पष्ट नहीं है कि मौजूदा डेटा की मदद से भविष्य का अनुमान लगा सके।
एसिम्प्टोमैटिक मामलों के बारे में ठीक ठीक अनुमान नहीं है। पिछले महीने सरकार के सीनियर वैज्ञानिक ने कहा था कि 100 में से 80 एसिम्प्टोमैटिक मामले हैं या फिर हल्के-फुल्के लक्षण वाले हैं।
सांख्यिकी विषय के प्रोफ़ेसर अतानु बिस्वास कहते हैं कि एसिम्प्टोमैटिक मामलों को शामिल करने के बाद भविष्य में लगाए जाने वाले आकलन पर बड़ा फर्क़ पड़ सकता है लेकिन आकड़ों के अभाव में कुछ निश्चित तौर पर कहा नहीं जा सकता है।
दूसरे कई विशेषज्ञों का कहना है कि हर रोज संक्रमण के मामले पता करना भी इस बात का सही संकेत नहीं है कि संक्रमण किस रफ़्तार से बढ़ रहा है।
पब्लिक हेल्थ फाउंडेशन के प्रेसिडेंट के श्रीनाथ रेड्डी का कहना है कि बेहतर विकल्प यह है कि हर रोज नए टेस्ट और नए मामलों पर नज़र रखा जाए। यह हमें कुछ हद तक संक्रमण के स्तर के बारे में बता सकता है।
उसी तरह से वो मानते हैं कि देश की आबादी की तुलना में कोरोना से होने वाले मौतों के दर का आकलन करना चाहिए क्योंकि आबादी बदलने वाला फैक्टर नहीं है। वो एक स्थायी फैक्टर है इसलिए यह एक बेहतर मापदंड होगा संक्रमण को मापने का।
पर्याप्त डेटा के अभाव में भारत में आने वाले समय में संक्रमण का स्तर का पता लगाना एक चुनौती भरा काम है। कितनी मौतें रिपोर्ट नहीं हुई हैं यह साफ़ नहीं है।
महामारी विशेषज्ञों का मानना है कि भारत में लक्षण वाले और बिना लक्षण वाले मरीजों के लिए ज़्यादा से ज़्यादा टेस्टिंग और कंट्रैक्ट ट्रैसिंग की ज़रूरत है। इसके साथ ही आइसोलेशन और क्वारंटीन सेंटर्स की भी ज़रूरत है।
यूनिवर्सिटी ऑफ मिशीगन के बायोस्टेटिक और ऐपिडॉमॉलॉजी के प्रोफेसर भ्रामार मुखर्जी कहते हैं, "हमें आने वाले कुछ हफ्तों के लिए विश्वसनीय मॉडल अपनाने की जरूरत है जो हमें भविष्य के बारे बता सके। हमें रोजमर्रा की ज़िंदगी में वायरस के जोख़िम को कम से कम कर के जीना सीखना होगा क्योंकि यह कहीं नहीं जा रहा, हमारे साथ ही रहेगा।"
इसके साथ ही कॉटैक्ट ट्रैसिंग के आधार पर हमें सुपर स्प्रेडर आयोजनों में शामिल लोगों, फ्रंटलाइनर्स, डिलिवरी और जरूरी सेवाओं में लगे लोगों के संपर्क में आने वालों की टेस्टिंग करनी होगी।
संक्रमण के सही-सही संख्या के बारे में बिना पता चले चले एक महामारी विशेषज्ञ के शब्दों में कहें तो भारत 'अंधेरे में तीर चलाता' रहेगा।
