कोरोना वायरस के संक्रमण की दूसरी लहर आख़िर कैसी होगी?

बायोलॉजिस्ट डॉक्टर जेनिफ़र रोन का कहना है कि कोरोना वायरस के संक्रमण की दूसरी लहर को लेकर सवाल यह नहीं है कि ये आएगी या नहीं। बल्कि, सवाल यह है कि ये कब आएगी और कितनी भयावह होगी।

डॉक्टर रोन इस बात पर नज़र बनाए हुए हैं कि ये महामारी एशिया से कैसे पैदा हुई और फिर पूरी दुनिया में फैल गई।

वर्ल्ड हेल्थ ऑर्गनाइज़ेशन (डब्ल्यूएचओ) ने कहा है कि कोरोना वायरस शायद हमारे बीच बना रहे और इसे कंट्रोल में लाने के लिए काफ़ी बड़े प्रयास करने पड़ेंगे।

यहां तक कि टेस्टिंग, ट्रेसिंग और लॉकडाउन मैनेजमेंट जैसी प्रभावी रणनीतियों के ज़रिए कोरोना वायरस से निबटने वाले एशिया के दक्षिण कोरिया, जापान और यूरोप में जर्मनी जैसे देशों में भी पाबंदियां हटने के बाद संक्रमण के नए मामले उभर रहे हैं।

इस हफ़्ते यूरोपीय यूनियन की कोविड-19 रिस्पॉन्स टीम ने सुझाव दिया है कि यूरोप को संक्रमण की दूसरी लहर के लिए तैयार रहना चाहिए।

गार्डियन अख़बार की रिपोर्ट के मुताबिक, टीम की डायरेक्टर एंड्रिया एमॉन कहती हैं कि अब सवाल यह है कि दूसरी लहर कब आएगी और कितनी बड़ी होगी।

पूरी दुनिया में सरकारें वायरस के दूसरे संभावित राउंड से टक्कर के लिए प्रावधान करने में जुट गई हैं। ऐसे में इनकी नजरें पूर्वी एशिया पर टिक गई हैं।

हम उन देशों से क्या सीख सकते हैं जिन्हें कोविड-19 से सबसे पहले जूझना पड़ा था और अब ये कोरोना वायरस के ग्राफ़ के मामले में दूसरों से आगे हैं?

डब्ल्यूएचओ के डायरेक्टर जनरल टेड्रोस कहते हैं कि हेल्थ एक्सपर्ट्स के मुताबिक, पूर्वी एशिया का बाकी की दुनिया के लिए मूल सबक हर केस को खोजना, आइसोलेट करना, टेस्ट करना और केयर करना है। साथ ही हर एक संपर्क को ट्रेस करना और क्वारंटीन करना भी ज़रूरी है।

डॉक्टर रोन यूनवर्सिटी कॉलेज लंदन में एक वायरस और सेल (कोशिका अध्ययन) एक्सपर्ट हैं। वह इस बात से सहमत हैं, उनका कहना है "एशिया से आ रहे आंकड़ों को देखकर पता चलता है कि आक्रामक तरीके से टेस्टिंग करना, ट्रेसिंग करना और फिर क्वारंटीन करना ही दूसरी लहर को कंट्रोल करने का एकमात्र तरीका है।"

मिसाल के तौर पर, साउथ कोरिया कभी कोविड-19 का हॉटस्पॉट था, लेकिन शुरुआत में ही सरकार ने बड़े पैमाने पर टेस्टिंग का सहारा लिया, साथ ही एप्स और जीपीएस टेक्नॉलॉजी का इस्तेमाल केसेज को ट्रेस करने के लिए किया गया।

डॉक्टर रोन कहती हैं कि, "इस रणनीति से उन्हें लोकल अलर्ट सिस्टम लगाने में मदद मिली। ऐसे में भले ही आम स्थिति कंट्रोल में हो लेकिन नया फोकस यह उभरा है कि किसी खास जगह को भी लॉकडाउन किया जा सकता है।"

डेटा एनालिसिस

एक्सपर्ट्स का कहना है कि दूसरा सबक यह है कि चीन, जापान और साउथ कोरिया जैसे देशों से आंकड़े इकट्ठे करने और उनकी रिसर्च करने की जरूरत है ताकि यह समझा जा सके कि यह वायरस किस तरह से व्यवहार करता है।

लंदन स्कूल ऑफ़ इकनॉमिक्स के स्वास्थ्य नीति विभाग की स्वास्थ्य अर्थशास्त्र की चेयर प्रोफेसर एलिस्टेयर मैकगुइर कहती हैं, "हमें अब रिकवरी रेट के बारे में कुछ जानकारियां मिल गई हैं। लेकिन, हमें अभी भी कॉन्टैक्ट रेट (संपर्क से संक्रमित होने के आसार) के बारे में काफी कुछ जानना है।"

यह एक नया वायरस है जो कि एशिया में सार्स और अफ्रीका में इबोला जैसे हाल में आए वायरसों से अलग तरह से व्यवहार करता है। ऐसे में इसको लेकर अभी कई तरह की जानकारियों को हासिल किया जाना बाकी है।

तीसरा सबक यह है कि पाबंदियों में ढील दिए जाने के बाद यह वायरस किस तरह से व्यवहार करता है। एशिया के अनुभव के आधार पर प्रोफेसर मैकगुइर कहती हैं, "बहुत ज्यादा आशावादी नहीं हुआ जा सकता है।"

एक सफल लॉकडाउन का यह अर्थ नहीं है कि कोई इलाका कोरोना से पूरी तरह से मुक्त हो गया है। जापान का होक्काइडो इलाका उन चुनिंदा जगहों में था जहां पर फरवरी के अंत में सबसे सख्त पाबंदियां लागू की गई थीं।

मार्च मध्य तक नए केस आना कम हो गए और यह संख्या एक या दो केस रोजाना पर सिमट गई। इन उपायों की सफलता ऐसी थी कि आपातस्थिति को हटा लिया गया और अप्रैल तक स्कूलों को खोल दिया गया।

किन, एक महीने से भी कम वक्त में आपातकालीन उपाय फिर से लागू करने पड़ गए क्योंकि यहां संक्रमण की दूसरी लहर तेजी से उठ खड़ी हुई थी।

डॉक्टर रोन कहती हैं, अब ऐसा होना कोई चौंकाने वाली बात नहीं रही है। वह कहती हैं, "यहां तक कि जिन देशों में महामारी कंट्रोल में दिख रही थी, वहां भी ढील दिए जाने के साथ ही संक्रमण में तेजी आने लगी है। यह पूरी दुनिया में हो रहा है।"

एक बार नहीं दो बार टेस्टिंग

स्वास्थ्य एक्सपर्ट एक सीधा सा संदेश दोहरा रहे हैं। डॉक्टर रोन के मुताबिक, "एशिया से हमें एक अहम चीज सीखने को मिली है वह यह है कि टेस्टिंग सबसे अहम है।"

एक्सपर्ट कहते हैं, "दक्षिण कोरिया जिस वजह से वायरस को रोकने में प्रभावी रहा वह उसकी आक्रामक टेस्टिंग, ट्रेसिंग और क्वारंटीन करने की पॉलिसी थी।"

शुरुआत में दक्षिण कोरिया में केस बढ़े। लेकिन, देश ने तेजी से एक ऐसा सिस्टम बना लिया जिसमें वहां रोज़ाना 10,000 टेस्ट मुफ्त में किए जाने लगे। यह बात फरवरी की है। 2015 में मर्स को लेकर उनके अनुभव ने उन्हें इसमें मदद दी।

नेशनल यूनिवर्सिटी ऑफ़ सिंगापुर में संक्रामक बीमारियों की प्रोफेसर ऊई एंग ओंग ने बीबीसी को मार्च में बताया था, "जिस तरह से उन्होंने कदम उठाए हैं और आबादी की स्क्रीनिंग की है वह वाकई में तारीफ़ के काबिल है।"

इसी तरह से जर्मनी ने भी एशिया की तर्ज पर अपने यहां मरने वालों का आंकड़ा काफी कम रखने में कामयाबी हासिल की। दूसरी ओर, यूके और स्पेन ऐसा नहीं कर पाए।

लेकिन, केवल ऐसा ही नहीं है। जिस तरह से एशिया ने आंकड़ों का प्रबंधन किया है उससे भी डबल टेस्टिंग की अहमियत साबित होती है।

प्रोफ़ेसर मैकगुइर के मुताबिक, "हमें केवल स्वाब टेस्ट से यह ही नहीं पता करना कि कौन संक्रमित हुआ है। आपको एक एंटीबॉडी टेस्ट भी करना होगा ताकि यह पता चले कि किसको यह हुआ था।"

मिसाल के तौर पर, ताइवान और जापान में जो लोग पॉजिटिव आए और वे जिन लोगों के संपर्क में आए थे उनका पता लगाया गया और उन्हें आइसोलेट किया गया। ऐसे में एक मैप उभकरकर आया जिससे यह पता चल रहा था कि संक्रमित लोग कहां हैं और कितनी तेजी से यह संक्रमण फैला है।

सिंगापुर ने सीसीटीवी फुटेज और दूसरे जरियों का इस्तेमाल कर हजारों लोगों को ट्रेस किया था। आइसोलेशन में मौजूद लोगों से दिन में कई दफा संपर्क किया जा रहा था और कई दफा जरूरत पड़ने पेर उन्हें अपनी लोकेशन का फोटोग्राफ प्रूफ भी देना पड़ता है।

हॉन्ग कॉन्ग ने तो और ज्यादा दखल देने वाले सिस्टम तैयार किए। विदेश से आने वालों के इलेक्ट्रॉनिक ब्रेसलेट्स लगाए गए थे।

एक्सपर्ट्स चेताते हैं कि जिन देशों ने बड़े पैमाने पर टेस्टिंग और ट्रेसिंग नहीं की है वहां जब संक्रमण की दूसरी लहर आएगी तब उनके पास जरूरी आंकड़ों का अभाव होगा।

प्रोफेसर मैकगुइर कहती हैं, "हम जानते हैं कि यह वाकई में एक बड़ी संक्रामक बीमारी है।"

सार्वजनिक स्वास्थ्य पर नए सिरे से फोकस

बार्सिलोना यूनवर्सिटी के स्कूल ऑफ इकनॉमिक्स की प्रोफेसर जुडिटवाल कहती हैं कि यह भी देखना जरूरी है कि सार्वजनिक स्वास्थ्य सेवाएं अपने अनुभव से क्या सबक सीख सकती हैं।

वह कहती हैं, "इस महामारी में हेल्थ सेक्टर ने यह साबित किया है कि खुद को नए सिरे से खड़ा कर सकता है और तेजी से अनुकूलन पैदा कर सकता है।"

चीन ने वुहान में 1,000 बेड वाला हॉस्पिटल महज आठ दिनों में खड़ा कर लिया। इस तरह से इस शहर ने यह बताया कि किस तरह से योजना बनाई जा सकती है और आपातकालीन स्थिति में हॉस्पिटल तैयार किए जा सकते हैं।

प्रोफ़ेसर वाल कहती हैं, "पूरी दुनिया के हॉस्पिटलों और प्राथमिक देखभाल केंद्रो ने दूसरों से काफी कुछ सीखा है, लेकिन उन्होंने खुद से भी बहुत कुछ सीखा है। और ऐसे में ये संक्रमण की दूसरी लहर आने पर उससे निबटने के लिए ज्यादा बेहतर स्थिति में होंगे।"

वह कहती हैं, "एशिया में अध्ययनों से पता चलता है कि इस तरह के अनुभवों से गुजरने के बाद स्वास्थ्य कर्मचारियों को भी पोस्ट-ट्रॉमेटिक स्ट्रेस डिसऑर्डर का शिकार होना पड़ सकता है।"

"पिछले आंकड़ों से पता चलता है कि सार्स के फैलने के करीब तीन साल बाद तक भी करीब 10 फीसदी स्टाफ में अवसाद के लक्षण दिखाई देते रहे"

महीनों तक एक के बाद एक लहर के दौर

महामारी विज्ञानी कह चुके हैं कि वायरस लहरों में चलते हैं। डॉक्टर रोन कहती हैं, "लेकिन, यह केवल एक लहर है जिसके लिए हम लॉकडाउन कर रहे हैं नहीं तो हमें एक बड़ा तबाही मचाने वाला दौर देखना पड़ेगा।"

वह कहती हैं, "संक्रमण तब वापसी करते हैं जब हम पाबंदियां हटा लेते हैं। जब आपका सामना एक नए वायरस से होता है और लोगों में इम्युनिटी नहीं होती है तब ऐसा ही होता है।"

एलएसई में स्वास्थ्य नीति विभाग की डॉक्टर लाइया मेनोऊ कहती हैं, "दूसरे देशों से हम केवल यही नहीं सीख सकते हैं, बल्कि हम गुजरे वक्त से भी सीख सकते हैं। 1918 में फैला स्पैनिश फ्लू ही पिछला एकमात्र ऐसा अनुभव है जिसका रिकॉर्ड है और जिसकी तुलना आज के वायरस से की जा सकती है।"

डॉक्टर मेनोऊ कहती हैं, "उस वक्त काफी सारा डेटा इकट्ठा किया गया था कि किस तरह से लॉकडाउन में ढील दी गई थी। पिछले आंकड़ों के आधार पर नए अध्ययन हमें अहम जानकारियां दे रहे हैं कि किस तरह से दूसरी लहर अलग-अलग आबादियों पर चोट करती है।"

डॉक्टर रोन कहते हैं, "1918 में पूरी दुनिया में एक के बाद एक लहर का दौर चला, यह चीज सख्त नीतियों पर आधारित थी।" "हम स्वाभाविक रूप से आशावादी हैं। लेकिन, फिलहाल सरकारों को लोगों की उम्मीदों को मैनेज करना होगा।"

इंतजार

वेस्टर्न पैसिफिक रीजन के लिए डब्ल्यूएचओ के कोविड-19 इंसीडेंट मैनेजर डॉक्टर नाओको इशिकावा कहते हैं कि लेकिन, शायद सबसे अहम सबक यह है कि कोई भी ऐसा उपाय या तरीका नहीं है जिसने अपने बूते कोई असर पैदा किया हो।

उन्होंने कहा, "यह केवल टेस्टिंग या केवल शारीरिक दूरी की पाबंदियों पर नहीं टिका है। इस क्षेत्र में कई देशों और इलाकों ने ये सभी चीजें एक व्यापक तौर पर की हैं।"

दूसरी लहरों के लिए कदम उठाना कई तरह के उपायों को एकसाथ करने पर आधारित है। डॉक्टर इशिकावा के मुताबिक, "कई उपाय वे हैं जो कि 2003 के सार्स के दौरान सीखे गए थे।"

डब्ल्यूएचओ ने वायरस पूरी तरह से गायब हो जाएगा, इसको लेकर अनुमान लगाने से बचने की चेतावनी दी है।

दक्षिण कोरिया और जापान का अनुभव बताता है कि इस वायरस को कंट्रोल करने के मैकेनिज्म किस तरह अनिश्चित है।
