तीसरी लहर की चेतावनियों के बीच भारत ने कोरोना वायरस वैक्सीन के उत्पादन की दिशा में तेज़ी से क़दम बढ़ाए हैं। भारत ने अब जॉनसन एंड जॉनसन की सिंगल डोज़ वैक्सीन के आपातकालीन इस्तेमाल को आख़िरकार मंज़ूरी दे दी है।
वैक्सीन 85% तक कारगर बताई जा रही है जो भारत की स्वदेशी वैक्सीन निर्माता कंपनी बायोलॉजिकल ई के साथ सप्लाई सौदे के तहत भारत को मिलेगी।
हालांकि अभी तक यह भी साफ़ नहीं है कि भारत को वैक्सीन कब तक मिल पाएगी, लेकिन जॉनसन एंड जॉनसन का कहना है कि आख़िरी तारीख़ के बारे में बताना काफ़ी जल्दी होगा।
भारत में अब तक कोविशील्ड, कोवैक्सीन और रूस की स्पूतनिक V वैक्सीन के इस्तेमाल को मंज़ूरी दी जा चुकी है और अब तक भारत में 50 करोड़ डोज़ दिए जा चुके हैं।
भारत में वैक्सीन
जॉनसन एंड जॉनसन की वैक्सीन भारत में ऐसी दूसरी विदेशी वैक्सीन है जिसके आपातकालीन इस्तेमाल की मंज़ूरी दी गई है।
इसकी अनुमति नई नीति के तहत दी गई है जिसके तहत उन निर्माताओं को अनुमति देने के लिए स्थानीय स्तर पर क्लीनिकल ट्रायल के आंकड़ों की की ज़रूरत नहीं है जिन्हें विश्व स्वास्थ्य संगठन या अमेरिका, यूरोपीय संघ, ब्रिटेन और जापान के विनियामक अनुमति दे चुके हैं।
जून में सरकार ने भारतीय फ़ार्मा कंपनी सिप्ला को मॉडर्ना वैक्सीन के आयात की अनुमति दी थी। इस वैक्सीन के कारगर होने की क्षमता 95 प्रतिशत तक पाई गई है, लेकिन वैक्सीन आने से पहले ही विवादों में फंस गई है।
दरअसल मॉडर्ना भारत में वैक्सीन के इस्तेमाल के दौरान किसी दावे की सूरत में क़ानूनी सुरक्षा चाह रही है और भारत सरकार ने इससे इनकार कर दिया है।
भारत में 3.2 करोड़ से अधिक कोरोना संक्रमण के मामले अब तक सामने आ चुके हैं, जबकि अमेरिका में अब तक 3.5 करोड़ मामलों का पता चला है।
भारत में दूसरी लहर की पीक के दौरान रोज़ाना 4 लाख के कऱीब मामले सामने आए थे लेकिन अब औसतन 30 से 40 हज़ार मामले सामने आने लगे हैं। विशेषज्ञों ने चेतावनी दी है कि तीसरी लहर से बचा नहीं जा सकता है।
दुनिया में मौतों के मामले में भारत तीसरे स्थान पर है जहां 4 लाख से अधिक मौतें हुई हैं। अमेरिका और ब्राज़ील ही ऐसे देश हैं जहां 4 लाख से अधिक मौतें हुई हैं।
टीकाकरण की हालत
सरकार का लक्ष्य है कि वो इस साल के अंत तक सभी वयस्क भारतीयों को वैक्सीन दे दे, लेकिन वैक्सीन की कमी, धीमी रफ़्तार और वैक्सीन लगवाने को लेकर हिचकिचाहट जैसी चुनौतियां भी सामने आ रही है।
जनवरी में शुरू हुए टीकाकरण से अब तक देश की 11% आबादी वैक्सीन के दोनों टीके ले चुकी है। वक़्त की भरपाई के लिए सरकार ने अब वैक्सीन के उत्पादन और उसकी ख़रीद में तेज़ी दिखाई है।
सरकार ने स्थानीय स्तर पर नोवावैक्स वैक्सीन के स्थानीय संस्करण के इस्तेमाल की तैयारी कर ली है जिसे सीरम इंस्टीट्यूट ऑफ़ इंडिया (SII) ने तैयार किया है। कंपनी का कहना है कि अमेरिका में क्लीनिकल ट्रायल के दौरान वैक्सीन 90% तक कारगर पाई गई है।
इसके अलावा भारत सरकार ने भारतीय कंपनी बायोलॉजिकल ई को 30 करोड़ वैक्सीन डोज़ का ऑर्डर दिया है।
नोवावैक्स वैक्सीन कब उपलब्ध होगी?
पिछले साल सितंबर में अमेरिकी दवा कंपनी नोवावैक्स ने सीरम इंस्टीट्यूट ऑफ़ इंडिया (SII) के साथ 2 अरब वैक्सीन डोज़ बनाने का सौदा किया था।
सीरम इंस्टीट्यूट ऑफ़ इंडिया के मुख्य कार्यकारी अधिकारी अदार पूनावाला को उम्मीद है कि नोवावैक्स का भारत में तैयार संस्करण 'कोवोवैक्स' सितंबर से उपबल्ध होगा।
उन्होंने कहा कि वैक्सीन के क्लीनिकल ट्रायल नवंबर तक पूरे होने की उम्मीद है, लेकिन सीरम इंस्टीट्यूट ऑफ़ इंडिया इसके पूरे होने से पहले ट्रायल के वैश्विक आंकड़ों के आधार पर लाइसेंस के लिए आवेदन कर सकता है।
नोवावैक्स वैक्सीन की दो डोज़ दी जाती हैं। अमेरिका में हुए ट्रायल के दौरान इस वैक्सीन को संक्रमण से अधिक गंभीर बीमार लोगों में इसे 91% और मध्यम और कम गंभीर मामलों में 100% असरदार पाया गया है।
बायोलॉजिकल ई वैक्सीन के बारे में कितना जानते हैं?
सरकार ने बायोलॉजिकल ई को 30 करोड़ वैक्सीन की ख़ुराक़ का ऑर्डर दिया है। इस कंपनी ने अमेरिकी दवा कंपनी डायनावैक्स और बेलर कॉलेज ऑफ़ मेडिसिन के साथ मिलकर वैक्सीन तैयार की है।
भारत ने 20।6 करोड़ डॉलर का ऑर्डर तब दिया है जब वैक्सीन को अभी तक आपातकालीन इस्तेमाल की मंज़ूरी नहीं मिली है।
सरकार ने अपने बयान में कहा कि यह अनाम वैक्सीन क्लीनिकल ट्रायल के तीसरे चरण में है, इसे हज़ारों लोगों को अब तक दिया जा चुका है और प्रभावकारिता और सुरक्षा के लिए परीक्षण किया गया है, शुरुआती दो चरणों में इसने 'आशाजनक परिणाम' पेश किए हैं।

स्पूतनिक V के बारे में कितना जानते हैं हम?
इस वैक्सीन को मॉस्को के गामालेया इंस्टीट्यूट ने तैयार किया है। यह तभी विवादों में घर गई थी जब अंतिम ट्रायल डाटा के सामने आए बिना ही इसे लगाया जाना शुरू कर दिया था। लेकिन वैज्ञानिकों का कहना है कि इसके फ़ायदे अब दिखाई दिए हैं।
इसमें सर्दी के एक वायरस का इस्तेमाल हुआ है जिसको हानिरहति बनाया गया है। यह कोरोना वायरस के एक छोटे अंश को शरीर में पहुंचाता है। वैक्सीन लगने के बाद शरीर एंटीबॉडी पैदा करना शुरू करता है जो कि ख़ासतौर से वायरल के अनुरूप होती है।
इसे 2 से 8 डिग्री सेल्सियस के तापमान पर स्टोर किया जा सकता है जो कि इसे एक जगह से दूसरी जगह ले जाने और इसे स्टोर करने में आसान बनाता है। हालांकि, बाक़ी वैक्सीन से अलग स्पूतनिक V की पहली और दूसरी वैक्सीन की ख़ुराक अलग-अलग होती है जो 21 दिनों के बाद दी जाती है।
ये दोनों ही कोरोना वायरस के ख़ास 'स्पाइक' को निशाना बनाती हैं लेकिन इसमें अलग-अलग वेक्टर इस्तेमाल होते हैं जो कि उस वायरस को बेअसर कर देते हैं जो शरीर में स्पाइक लाता है।
इसका आइडिया यह माना गया है कि एक ही वैक्सीन के एक वर्ज़न को दो बार इस्तेमाल करने से बेहतर दो विभिन्न फॉर्मूला इस्तेमाल करना है और यह लंबी सुरक्षा दे सकता है।
भारत को 12।5 करोड़ वैक्सीन की डोज़ की पहली खेप मई में मिली थी। रिपोर्ट्स के अनुसार, छह घरेलू वैक्सीन निर्माताओं के साथ 75 करोड़ वैक्सीन की खुराक बनाने के लिए इस वैक्सीन की मार्केटिंग करने वाले रशियन डायरेक्ट इन्वेस्टमेंट फंड (RDIF) ने समझौता किया है।
स्पूतनिक V को अर्जेंटीना, फ़लस्तीनी प्राधिकरण, वेनेज़ुएला, हंगरी, संयुक्त अरब अमीरात और ईरान समेत 60 देश में अनुमति दी जा चुकी है।

कोवैक्सीन के बारे में हम क्या जानते हैं?
कोवैक्सीन एक इनएक्टिवेटेड वैक्सीन है जिसका अर्थ है कि यह मारे गए कोरोना वायरस से बनाई गई है जो कि सुरक्षित तरीक़े से शरीर में पहुंचाई जा सकती है।
24 साल पुरानी वैक्सीन निर्माता कंपनी भारत बायोटेक ने इसे तैयार किया है। भारत बायोटेक ने नेशनल इंस्टीट्यूट ऑफ़ वायरोलॉजी के द्वारा उपलब्ध कराए गए कोरोना वायरस के ख़ास सैंपल का इस्तेमाल किया है।
इम्यून सेल्स मरे हुए वायरस की पहचान करते हैं और यह इम्यून सिस्टम को महामारी के वायरस के ख़िलाफ़ एंटीबॉडीज़ बनाने लगते हैं।
इसमें दो ख़ुराक को चार सप्ताह के अंतराल के बाद दिया जाता है। वैक्सीन को 2 डिग्री सेल्सियस से 8 डिग्री सेल्सियस के बीच रखा जा सकता है।
शुरुआत से लेकर तीसरे चरण के डाटा तक इस वैक्सीन को वायरस पर 81% तक कारगर पाया गया है।
तीसरे चरण का ट्रायल जब जारी ही था तब भारत के नियामक ने जनवरी में ही वैक्सीन के आपातकालीन इस्तेमाल को मंज़ूरी दे दी थी। इसके बाद विशेषज्ञों ने इस पर सवाल खड़े किए थे।
कोवैक्सीन को लेकर क्या विवाद था?
यह विवाद तब शुरू हुआ था जब विनियामक ने क्लीनिकल ट्रायल चरण के दौरान इसके 'भरपूर एहतियात के साथ आपातकालीन स्थिति में जनता के हित के लिए सीमित इस्तेमाल की अनुमति दे दी गई थी।'
विशेषज्ञों ने तब इस बात पर हैरत जताई थी कि एक वैक्सीन के करोड़ों लोगों के लिए आपातकालीन इस्तेमाल की मंज़ूरी कैसे दी जा सकती है जबकि इसका ट्रायल अभी जारी है। द ऑल इंडिया ड्रग एक्शन नेटवर्क ने उस समय कहा था कि 'पूर्ण रूप से बिना अध्ययन किए गए टीके' को अनुमति देना 'वैज्ञानिक तर्क के समझ के परे है।'
वैक्सीन निर्माता और दावा विनियामक ने कोवैक्सीन का बचाव किया और कहा कि यह 'सुरक्षित है और एक मज़बूत प्रतिरक्षा प्रतिक्रिया देता है।'
भारत बायोटेक ने कहा था कि भारतीय क्लीनिक ट्रायल का क़ानून 'देश में गंभीर और जानलेवा बीमारियों के दौरान चिकित्सीय ज़रूरतों को पूरा करने के लिए' दूसरे चरण के ट्रायल के बाद दवा के 'तेज़ी' से अनुमति दिए जाने की अनुमति देता है।
कंपनी ने वादा किया था कि वह जुलाई में तीसरे चरण के ट्रायल के बाद पूरा डाटा मुहैया कराएगी।

कोविशील्ड क्या है?
ऑक्सफ़ोर्ड-एस्ट्राज़ेनेका की वैक्सीन को स्थानीय स्तर पर SII बना रहा है।
इस वैक्सीन को चिंपेंज़ी के सामान्य सर्दी के वायरस (एडीनो वायरस) के कमज़ोर वर्ज़न से बनाया गया है। इसको कोरोना वायरस जैसा बनाया गया है हालांकि यह बीमारी का कारण नहीं बनता है।
वैक्सीन को जब मरीज़ के शरीर में इंजेक्शन के ज़रिए पहुंचाया जाता है तो यह शरीर के इम्युन सिस्टम को वैक्सीन बनाने के संकेत देता है और कोरोना वायरस संक्रमण के ख़िलाफ़ लड़ता है।
इस वैक्सीन को 2 डिग्री सेल्सियस से 8 डिग्री सेल्सियस के तापमान पर सुरक्षित रखा जा सकता है और इसकी दो ख़ुराक को चार से 12 सप्ताह के अंतराल पर दिया जाता है।
कोविशील्ड कितनी असरदार है?
ऑक्सफ़ोर्ड-एस्ट्राज़ेनेका वैक्सीन के अंतरराष्ट्रीय क्लीनिकल ट्रायल बताते हैं कि जब लोगों को आधी ख़ुराक के बाद अगली पूरी ख़ुराक दी गई तो यह 90% तक असरदार थी।
हालांकि, आधी ख़ुराक और पूरी ख़ुराक के आइडिया को पुष्टि करने के लिए कोई साफ़ आंकड़े मौजूद नहीं हैं।
इसके अलावा एक अप्रकाशित डाटा बताता है कि दो ख़ुराक के बीच लंबा अंतर रखने से इसका असर काफ़ी बढ़ जाता है। एक ग्रुप को जब वैक्सीन दी गई तो पहली ख़ुराक के बाद यह 70% तक असरदार थी।
भारत में इस वैक्सीन को बनाने वाली कंपनी का कहना है कि कोविशील्ड 'बहुत असरदार' है और ब्राज़ील और ब्रिटेन के तीसरे ट्रायल के आंकड़े इसका समर्थन करते हैं।
क्लीनिकल ट्रायल तीन चरण की प्रकिया होती है जिसमें वैक्सीन का प्रभाव पता चलता है कि कहीं यह कभी साइड अफ़ेक्ट्स तो नहीं पैदा कर रही है।
कितनी और वैक्सीन हैं कतार में?

भारत की वैक्सीन कौन से देश ले रहे हैं?
भारत ने वैक्सीन की 6.6 करोड़ ख़ुराक को लैटिन अमेरिका, कैरिबियाई, एशिया और अफ़्रीका के 95 देशों को भेजा है। ब्रिटेन, कनाडा, ब्राज़ील और मेक्सिको को यह वैक्सीन मिली है।
कोविशील्ड और कोवैक्सीन का निर्यात किया गया है। इनमें से कुछ 'तोहफ़े' के तौर पर और कुछ कोवैक्स योजना के तौर पर दी गई हैं। विश्व स्वास्थ्य संगठन की कोवैक्स योजना के तहत एक साल के अंदर 190 देशों को 2 अरब वैक्सीन की डोज़ दी जानी हैं।
लेकिन मार्च में भारत ने ऑक्सफ़ोर्ड-एस्ट्राज़ेनेका वैक्सीन के सभी निर्यात पर अस्थाई रोक लगा दी थी। सरकार का कहना था कि देश में लगातार बढ़ते संक्रमण के मामलों के दौरान इसकी मांग बढ़ सकती है इसलिए इन ख़ुराक की भारत में ही ज़रूरत है।
