क्या सेक्स की लत वाक़ई एक बीमारी है?

अमेरिकी लेखक मार्क ट्वेन का एक कथन प्रचलित है। उन्होंने कहा था, "सिगरेट छोड़ना आसान था, मैंने ऐसा सौ बार किया।" हालांकि बहुत मुमकिन है उन्होंने ऐसा कभी ना कहा हो। बाद में उनकी मौत फेफड़ों के कैंसर से हो गई थी।

एक समाज के तौर पर हमने निकोटीन, शराब और ड्रग्स जैसी नशीली चीज़ों की लत को स्वीकार किया है। हमने इनसे होने वाले नुक़सान को भी स्वीकार किया है। लेकिन अगर बात सेक्स की लत की हो तो विशेषज्ञों की राय बंटी हुई है। कुछ का मानना है कि सेक्स की लत होती है और कुछ इसे नकारते हैं।

सेक्स की लत फ़िलहाल बीमारी नहीं है और इसलिए अभी कितने लोगों ने इसे लेकर चिकित्सकीय परामर्श मांगा है, इसका अधिकारिक डेटा भी उपलब्ध नहीं है। पोर्न और सेक्स की लत से जूझ रहे लोगों की मदद के लिए विकसित की गई एक वेबसाइट ने ब्रिटेन में 21,000 लोगों पर सर्वे किया। इन लोगों ने साल 2013 के बाद से वेबसाइट से मदद के लिए संपर्क किया था। इनमें से 91 प्रतिशत पुरुष थे और सिर्फ़ दस ने ही अपनी परेशानी के लिए डॉक्टर से परामर्श लिया था।

विशेषज्ञों की राय

सेक्स की लत को 2013 के 'डायग्नोस्टिक एंड स्टेटिस्टिकल मैनुअल ऑफ़ मेंटल डिसआर्डर्स' (डीएसएम) में शामिल करने पर विचार किया गया था, लेकिन सबूतों के अभाव में इसे शामिल नहीं किया गया। डीएसएम अमरीका और ब्रिटेन में एक अहम डायग्नोस्टिक टूल है। लेकिन अब कंपल्सिव सेक्शुअल बिहेवियर (बाध्यकारी यौन व्यवहार) को विश्व स्वास्थ्य संगठन की ओर से जारी होने वाले 'मैनुअल इंटरनेशनल क्लासीफ़िकेशन ऑफ़ डिसीज़' (आईसीडी ) में शामिल करने पर विचार किया जा रहा है।

नए सबूत सामने आने के बाद जुआ खेलने की लत और ज़्यादा खाने की लत को भी 2013 में बीमारी के रूप में स्वीकार कर लिया गया था। पहले जुए को बाध्यकारी व्यवहार ही माना जाता था। थेरेपिस्टों का मानना है कि सेक्स की लत भी ऐसे ही इसमें शामिल हो सकती है।

दिमाग़ में क्या चलता है

एक शोध से पता चलता है कि सेक्स की लत का शिकार कोई व्यक्ति जब पोर्न देखता है तो उसके दिमाग़ में ऐसी ही गतिविधियां होती हैं जैसी किसी नशे की लत के शिकार व्यक्ति के दिमाग़ में ड्रग्स दिखने पर होती है। किसी व्यक्ति को सेक्स की लत है या नहीं - ये इस बात पर निर्भर करता है कि आप किस चीज़ को लत मानते हैं और इसकी कोई निर्धारित आधिकारिक व्याख्या नहीं है।

ओपन यूनिवर्सिटी में एमेरिटस प्रोफ़ेसर डॉ। फ़्रेडरिक टोएट्स कहते हैं, "अगर ये कोई ऐसी चीज़ है जिस पर कोई शारीरिक रूप से निर्भर है और जिसे छोड़ने पर शारीरिक चोट पहुंचेगी तो फिर सेक्स लत नहीं हो सकती है।" हालांकि उनका मानना है कि विस्तृत व्याख्या अधिक मददगार होगी।

डॉ. टोएट्स कहते हैं कि किसी भी लत की दो पहचान होती हैं- आनंद या रिवॉर्ड की चाह और इस व्यवहार के इर्द-गिर्द संघर्ष का होना। विशेषज्ञों का मानना है कि प्रतिफल की चाह ही लत को बाध्यकारी व्यवहार से अलग करती है। हालांकि दोनों में बहुत हद तक समानताएं हैं।

प्रोफ़ेसर टोएट्स कहते हैं, ''लत के शिकार लोग अल्पकालिक फ़ायदा देखते हैं भले ही दीर्घकाल में उन्हें नुक़सान ज़्यादा हो रहा हो। इसके विपरीत बाध्यकारी व्यवहार (आब्सेसिव कंपल्सिव डिसऑर्डर) से प्रभावित लोग ऐसा व्यवहार भी करते हैं जिससे उन्हें कोई मज़ा नहीं मिलता।''

लेकिन आनंद की चाह तो हम सबमें होती है तो फिर सामान्य प्रतिफल की चाह वाले व्यवहार और लत में क्या अंतर होता है?

मनोवैज्ञानिक डॉ. हैरियट गैरॉड मानती हैं कि कोई व्यवहार तब लत बन जाता है जब वो इनता तीव्र हो जाता है कि इससे व्यक्ति और उसके इर्द-गिर्द मौजूद लोगों को नुक़सान पहुंचने लगे। वो कहती हैं कि जुआ खेलने या अधिक खाने की लत को बीमारी मान लिया गया है, लेकिन सेक्स की लत को नहीं क्योंकि वो जनमानस में लंबे समय से हैं। इसका मतलब ये है कि जुए और अधिक खाने के मामले में ज़्यादा लोग मदद लेने के लिए सामने आए हैं जिससे उनको बीमारी होने के समर्थन में अधिक सबूत मिले हैं।

डॉ. अबिगेल सान क्लिनिकल साइकॉलजस्ट हैं जो मानती हैं कि सेक्स संबंधी व्यवहार की भी लत हो सकती है, लेकिन जो लोग इस भावना के नियंत्रण से बाहर होने का सामना कर रहे हैं उनके लिए सेक्स किसी और छुपी हुई समस्या की वजह भी हो सकता है जैसे कि वो अवसाद, चिंता या आघात से उबरने के लिए सेक्स का सहारा लिया जाना।

वो कहती हैं, "अलग-अलग गतिविधियां और मादक पदार्थों से जुड़ी गतिविधियां अलग-अलग तरीके से प्रतिफल देती हैं, लेकिन बात ये है कि वो प्रतिफल देती हैं। ऐसा मानने का कोई कारण नहीं है कि सेक्स भी इस तरह से काम न करता हो, लेकिन बात ये है कि इसके अभी हमारे पास पर्याप्त सबूत नहीं हैं।"

हालांकि वो इस बात से संतुष्ट नहीं हैं कि सेक्स को लत मान लेने से लोगों की मदद होगी, ख़ासकर उनकी जो सेक्स का इस्तेमाल दूसरी समस्याओं से निबटने के लिए कर रहे हैं। उन्हें लगता है कि इससे ओवर डायग्नोसिस की समस्या पैदा हो सकता है यानी ग़लत दवाइयां दी जाएं या लक्षण को ठीक से एनालाइज़ न किया जाए।

क्या सेक्स की लत एक मिथक है?

कई लोग इस बात को स्वीकार नहीं करते हैं कि सेक्स की लत एक वास्तविक बीमारी है। 'द मिथ ऑफ़ सेक्स एडिक्शन' किताब लिखने वाले सेक्स थेरेपिस्ट डेविड ले कहते हैं कि 'आम तौर पर सेक्स की लत मान लिया जाने वाला व्यवहार वास्तव में मूड और कुंठा से जुड़ी समस्याओं का इलाज न कराने का लक्षण होता है।'

वो कहते हैं, "सेक्स या हस्तमैथुन को शराब या ड्रग्स के बराबर मानना सही नहीं है। शराब की लत के शिकार लोग इसे छोड़ने से मर भी सकते हैं।" वो कहते हैं, "सेक्स की लत की धारणा स्वस्थ यौन संबंध क्या है इससे जुड़े नैतिक मूल्यों पर आधारित है।"

"अगर आप अपने थेरेपिस्ट की समझ या धारणा से ज़्यादा या अलग तरीके से सेक्स कर रहे हैं तो आप उनकी नज़र में सेक्स की लत के शिकार हो सकते हैं!" इंटरनेशनल क्लासिफ़िकेशन ऑफ़ डिसीज़ के अगले अंक में कंपल्सिव सेक्शुअल बिहेवियर को शामिल करने पर लिखे शोध पत्र में शोधकर्ताओं के एक समूह ने तर्क दिया है कि इस जाल से बचा जाना चाहिए।

उनका कहना है कि "सेक्स में अधिक रुचि या व्यवहार" का "यौन इच्छाओं के बारे में नैतिक निर्णयों या सेक्स की इच्छा को अस्वीकार किए जाने" के आधार पर इलाज नहीं किया जाना चाहिए। हालांकि जो लोग इस स्थिति को औपचारिक रूप से स्वीकार किए जाने की मांग कर रहे हैं उनका मानना है कि इससे वो लोग मदद लेने के लिए आगे आ सकेंगे जो परेशानी का सामना कर रहे हैं, भले ही सेक्स की लत अपने आप में एक समस्या हो या फिर अन्य किसी ग़हरी समस्या का लक्षण हो।
