आठ साल की बच्ची और दिमाग़ में 100 से ज्यादा कीड़े के अंडे। माता-पिता के लिए समझ पाना बहुत मुश्किल था कि आख़िर क्यों उनकी बेटी लगभग हर रोज़ सिर दर्द की शिकायत करती है। क्यों उसे रहते-रहते दौरे आने लगते हैं।
लगभग 6 महीने से ऐसा चल रहा था, लेकिन जब इसकी वजह पता चली तो उन्हें यक़ीन नहीं हुआ। "बच्ची के दिमाग़ में 100 से ज़्यादा टेपवर्म यानी फ़ीताकृमी के अंडे थे। जो दिमाग़ में छोटे-छोटे क्लॉट (थक्के) के रूप में नज़र आ रहे थे। "
गुड़गांव स्थित फ़ोर्टिस अस्पताल में न्यूरोलॉजी डिपार्टमेंट के डायरेक्टर डॉ. प्रवीण गुप्ता की देखरेख में बच्ची का इलाज चल रहा है। डॉ. गुप्ता बताते हैं "हमारे पास आने से पहले वो इलाज करवा रही थी। उसे तेज़ सिर दर्द की शिकायत थी और दौरे पड़ते थे। वो दिमाग़ में सूजन और दौरे पड़ने का ही इलाज करवा रही थी।"
बच्ची के दिमाग़ की सूजन कम करने के लिए लिए बच्ची को स्टेरॉएड्स दिया जाने लगा था। इसका असर ये हुआ कि आठ साल की बच्ची का वज़न 40 किलो से बढ़कर 60 किलो हो गया। वज़न बढ़ा तो और तक़लीफ़ बढ़ गई। चलने-फिरने में दिक्क़त आने लगी और सांस लेने में तक़लीफ़ शुरू हो गई। वो पूरी तरह स्टेरॉएड्स पर निर्भर हो चुकी थी।
बच्ची जब डॉ. गुप्ता के पास आई तो उसका सिटी-स्कैन किया गया। जिसके बाद उसे न्यूरोसिस्टिसेरसोसिस से पीड़ित पाया गया। डॉक्टर गुप्ता बताते हैं "जिस समय बच्ची को अस्पताल लाया गया वो होश में नहीं थी। सिटी स्कैन में सफ़ेद धब्बे दिमाग़ में नज़र आए। ये धब्बे कुछ और नहीं बल्कि फ़ीताकृमी के अंडे थे। वो भी एक या दो नहीं बल्कि सौ से ज़्यादा की संख्या में।"
जब बच्ची डॉ. गुप्ता के पास पहुंची तो उसके दिमाग पर प्रेशर बहुत अधिक बढ़ चुका था। अंडों का प्रेशर दिमाग़ पर इस कदर हो चुका था कि उसके दिमाग़ ने काम करना बंद कर दिया था।
डॉ. गुप्ता बताते हैं, "सबसे पहले तो हमने दवाइयों से उसके दिमाग़ का प्रेशर (दिमाग़ में कोई भी बाहरी चीज़ आ जाए तो इससे दिमाग का अंदरूनी संतुलन बिगड़ जाता है) कम किया। उसके बाद उसे सिस्ट मारने की दवा दी गई। ये काफी ख़तरनाक भी होता है क्योंकि इस दौरान दिमाग़ का प्रेशर बढ़ भी सकता है।"
फ़िलहाल अंडों को ख़त्म करने की पहली ख़ुराक बच्ची को दी गई है, लेकिन अभी सारे अंडे ख़त्म नहीं हुए हैं। डॉ. गुप्ता बताते हैं दिमाग़ में ये अंडे लगातार बढ़ते रहते हैं। ये अंडे सूजन और दौरे का कारण बनते हैं।

पर दिमाग़ तक पहुंचे कैसे ये अंडे?
डॉ. गुप्ता बताते हैं कि कोई भी चीज़ जो अधपकी रह जाए तो उसे खाने से, साफ़-सफ़ाई नहीं रखने से टेपवॉर्म पेट में पहुंच जाते हैं। इसके बाद ख़ून के प्रवाह के साथ ये शरीर के अलग-अलग हिस्सों में चले जाते हैं।
"भारत में मिर्गी के दौरे की एक जो बड़ी परेशानी है उसका एक प्रमुख कारण टेपवर्म है। भारत में टेपवर्म का संक्रमण बहुत ही सामान्य है। क़रीब 12 लाख लोग न्यूरोसिस्टिसेरसोसिस से पीड़ित हैं, जो मिर्गी के दौरों का एक प्रमुख कारण है।"
टेपवर्म है क्या?
टेपवर्म एक तरह का पैरासाइट है। ये अपने पोषण के लिए दूसरों पर आश्रित रहने वाला जीव है। इसलिए ये शरीर के अंदर पाया जाता है, ताकि उसे खाना मिल सके। इसमें रीढ़ की हड्डी नहीं होती है।
इसकी 5000 से ज़्यादा प्रजातियां पाई जाती हैं। ये एक मिमी से 15 मीटर तक लंबे हो सकते हैं। कई बार इसका सिर्फ़ एक ही आश्रय होता है तो कई बार एक से अधिक। इसका शरीर खंडों में बंटा होता है।
इसके शरीर में हुक के जैसी संरचनाएं होती हैं जिससे ये अपने आश्रयदाता के अंग से चिपका रहता है। शरीर पर मौजूद क्यूटिकिल की मदद से यह अपना भोजन लेता है। यह पचा-पचाया भोजन ही लेते हैं क्योंकि इनमें पाचन-तंत्र नहीं होता है।
कैसे फैलता है ये?
टेपवर्म फ़्लैट, रिबन के जैसी संरचना वाले होते हैं। अगर फ़ीताकृमी का अंडा शरीर में प्रवेश कर जाता है तो यह आंत में अपना घर बना लेता है। हालांकि ज़रूरी नहीं कि ये पूरे जीवनकाल आंत में ही रहे, खून के साथ ये शरीर के दूसरे हिस्सों में भी पहुंच जाता है।
लीवर में पहुंचकर ये सिस्ट बना लेते हैं, जिससे पस हो जाता है। कई बार ये आंखों में भी आ जाते हैं और दिमाग़ में भी। एशिया की तुलना में यूरोपीय देशों में इसका ख़तरा कम है। एनएचएस के अनुसार, अगर शरीर में टेपवर्म है तो ज़रूरी नहीं कि इसके कुछ लक्षण नज़र ही आए, लेकिन कई बार ये शरीर के कुछ अति-संवेदनशील अंगों में पहुंच जाता है, जिससे ख़तरा हो सकता है।
हालांकि इसका इलाज भी आसान है। दिल्ली स्थित सर गंगाराम अस्पताल में गैस्ट्रोलॉजिस्ट डॉ. नरेश बंसल के अनुसार, भले ही टेपवर्म जानलेवा नहीं हैं, लेकिन इन्हें नज़रअंदाज़ करना ख़तरनाक हो सकता है। डॉ. बंसल मानते हैं कि यूं तो टेपवर्म दुनिया भर में पाए जाते हैं और इनसे जुड़ी स्वास्थ्य समस्याएं भी लेकिन भारत में इसके संक्रमण से जुड़े मामले ज़्यादा सामने आते हैं।

टेपवर्म के कारण
- अधपका या कच्चा पोर्क या बीफ़ खाने से, अधपकी या कच्ची मछली के सेवन से। दरअसल, इन जीवों में टेपवर्म का लार्वा होता है। ऐसे में अगर इन्हें अच्छी तरह पका कर नहीं खाया जाए तो टेपवर्म शरीर में पहुंच जाते हैं।
*दूषित पानी पीने से।
*पत्ता-गोभी, पालक को अगर अच्छी तरह पकाकर नहीं बनाया जाए तो भी टेपवर्म शरीर में पहुंच सकता है।
*इसलिए गंदे पानी में या मिट्टी के संपर्क में उगने वाली सब्जियों को धो कर खाने की सलाह दी जाती है।
टेपवर्म संक्रमण के लक्षण
आमतौर पर इसका कोई बहुत सटीक लक्षण नज़र नहीं आता है, लेकिन टेपवर्म शरीर में हों तो शौच से पता चल जाता है। इसके अलावा पेट में दर्द, डायरिया, कमज़ोरी और उल्टी, अनियमित भूख और कमज़ोरी इसके प्रमुख लक्षण हैं। अगर शरीर में टेपवर्म की संख्या या अंडों की संख्या बहुत अधिक है तो चक्कर आना, त्वचा का पीलापन, खांसी, सांस फूलना, देखने में दिक्क़त जैसी शिकायतें भी हो सकती हैं।
बचाव के उपाय
टेपवर्म एकबार शरीर में पहुंच जाए तो इससे दवा की मदद से ही छुटकारा पाया जा सकता है। लेकिन अगर कुछ सावधानियां बरती जाएं तो इसके संक्रमण से बचा जा सकता है।
*किसी भी किस्म के मांस को बिना अच्छी तरह पकाए न खाएं।
*फल-सब्जियों को खाने से पहले अच्छी तरह धो लें।
*खाना खाने से पहले हाथ ज़रूर धोएं। शौच के बाद हाथों और नाखूनों को अच्छी तरह साफ़ करें।
*हमेशा साफ़ पानी ही पिएं।
*मवेशियों के सीधे संपर्क से बचें या उस दौरान विशेष सावधानी रखें।
डॉ. बंसल मानते हैं कि टेपवर्म जानलेवा नहीं है ये मानकर इसे लापरवाही में नहीं लेना चाहिए। ये शरीर के किसी ऐसे अंग में भी जा सकता है, जिससे शरीर का वो हिस्सा लकवाग्रस्त हो सकता है।
