डॉ भारत रावत का कहना है कि सभी जगह इस इंजेक्शन का उपयोग जरूरत से ज्यादा होने लगा। वहीं लोगों के मन में यह बात बैठ गई कि रेमडेसिविर इंजेक्शन लग गया तो हम ठीक ही हो जाएंगे। कई जगह पर लोग अपने मन से ही इसका इलाज शुरू करने लगे हैं।
रेमडेसिविर नहीं मिलने से मौत?
एक स्टडी में सामने आया है कि रेमडेसिविर इंजेक्शन देने से जान बच जाएंगी या नहीं देने से जान चली जाएं, यह वैज्ञानिक तौर पर कहना मुश्किल है। यह जरूर देखा गया है कि बीमारी का समय जरूर घटा है। पहले ठीक होने में 10 से 15 दिन लगते थे, लेकिन अब 6 से 7 दिन में ठीक हो जाते हैं। लेकिन मत्यू दर में ऐसी कोई कमी नहीं देखी गई है।
यह कहना गलत है कि रेमडेसिविर की कमी से इतनी मौत हो रही है। वर्तमान में संक्रमण बहुत तेजी से फैल रहा है। पहले की तुलना में संक्रमण से मरने वालों के आंकड़ें भी बढ़ गए है।
रेमडेसिविर कब दिया जाता है?
यह इंजेक्शन वायरस के लोड को कम करता है। इसे बीमारी के शुरूआती दौर में दिया जाता है। जिन मरीजों का बुखार ठीक नहीं हो रहा है या जिनके ब्लड में सीआरपी बढ़ जाता है। उन्हें रेमडेसिविर दिया जाता है। पहले दिन इसके दो डोज दिए जाते हैं और फिर 4 दिन एक- एक डोज दिए जाते हैं।
डेक्सामेथासोन स्टेरॉयड के बारे में?
इस दवाई का उपयोग चौथे दिन से किया जाता है। इससे फेफड़ों में कोविड संक्रमण का असर घटता है। यह दवाई भी सिर्फ उन्हें ही दी जाती है जिनके अंदर संक्रमण बढ़ गया हो।
क्या दवा सस्ती होने के कारण इसे रिकमंड नहीं किया जा रहा?
डेक्सामेथासोन दूसरी दवाईयों की तुलना में सस्ती है। इसका उपयोग हो रहा है, लेकिन कम। यह बात सही है कि दवाई सस्ती है। भीड़ की भी एक मानसिकता होती है। जहां भीड़ अधिक होती है वहां झुकाव ज्यादा होता है।
क्या डेक्सामेथासोन अधिक कारगर है? लोग रेमडेसिविर के पीछे भाग रहे हैं?
फिलहाल तुलना करना सही नहीं है। दोनों अलग- अलग तरह से काम कर रही है। कई बार दोनों दवाओं के डोज भी एक साथ दिए जा रहे हैं।
मरीजों के लिए आपकी सलाह?
सभी मरीज और उनके परिजनों से कहना चाहूंगा कि अगर प्रारंभिक लक्षण है, बुखार सिर्फ 1 या 2 दिन है, ऑक्सीजन सैचुरेशन लेवल मेंटेन है। 94 परसेंट या उससे अधिक है तो आप घबराएं नहीं। आपको सिर्फ अच्छी नींद लेना है, 3 लीटर पानी पीना है और खुश रहना है।
हां, तीसरे दिन भी आपका बुखार नहीं उतरता है, ऑक्सीजन सैचुरेशन लेवल 94 से कम होता है, तब आपको जरूर डॉ से सलाह लेना चाहिए। अगर आप डॉ को नहीं दिखा पा रहे हैं तो ऑनलाइन भी डॉ से बात कर सकते हैं।

इन दोनों दवाइयों में समय का अपना महत्व है। इन दोनों दवाओं में से लाइफ सेविंग कोई भी नहीं है। हां, दोनों दवाइयों को सही समय पर देने से परिणाम बेहतर हो सकते हैं। हमें रिजल्ट्स मिले भी हैं।
डेक्सामेथासोन और रेमडेसिविर में क्या फर्क, कौन सा बेहतर?
जब सिवियर कंडीशन हो जाती है तब रेमडेसिविर ही मददगार है। विशेषज्ञों के अनुसार जब सिवियर स्टेज पर कोई दवा काम नहीं करेगी और दूसरा कोई ऑप्शन भी नहीं है। उस स्थिति में रेमडेसिविर दवा की मंजूरी दी गई है। वहीं रेमडेसिविर को सिर्फ 5 से 6 दिन के भीतर इस्तेमाल करना ही सही तरीका है।
एम्स की गाइडलाइन के निर्देशानुसार कहा गया कि गंभीर स्थिति में आप रेमडेसिविर दे सकते हैं। 7 से अधिक दिन भी दे सकते हैं। अलग - अलग कंसलटेंट, अलग- अलग डॉ के समूह और कहीं न कहीं अलग- अलग स्टेटमेंट की वजह से कन्फ्यूजन की स्थिति पैदा हो गई है। चूंकि अभी कोई दूसरी एंटी वायरल है भी नहीं जिस पर बहुत अधिक शोध किया गया है। ऐसे में इस गंभीर स्थिति में यही ऑप्शन है।
क्या रेमडेसिविर के अभाव में मौतें हो रही हैं?
यह गलत है कि रेमडेसिविर इंजेक्शन नहीं लगेगा तो जान नहीं बचेगी। कई लोग इसके बिना भी ठीक हुए हैं।
मरीजों के लिए आपकी सलाह?
आप सावधान रहे। हर मरीज को भर्ती होने की जरूरत नहीं है। हर मरीज को रेमडेसिविर की आवश्यकता नहीं है। अगर देखा जाए तो 100 में से 80 फीसदी मरीज होम आइसोलेशन में ठीक हो सकते हैं। आज के वक्त में संक्रमण फैलने का सबसे बड़ा कारण है हमने सोशल डिस्टेंसिंग छोड़ दी, सभी लोग केयरलेस हो गए। वहीं हमारा आंकलन है कि लोग थक गए है। ऐसा अब कब तक चलेगा। क्योंकि यह सभी चीजें लोगों को गलत तरीके से झेलना पड़ रही है। वैक्सीनेशन के बाद लोगों को लगने लगा कि अब हमें कोविड नहीं होगा। जबकि यह सच नहीं है। वैक्सीन के बाद कोविड के सभी नियमों का पालन करना है। क्योंकि आने वाले 4 से 5 महीने में सभी को हो सकता है। इसलिए सभी जल्द से जल्द वैक्सीन लगवाएं।
रेमडेसिविर इंजेक्शन पर विशेषज्ञों का तर्क
इन दिनों पूरा देश रेमडेसिविर इंजेक्शन की मांग से जूझ रहा है। वहीं विशेषज्ञों का कहना है कि इस इंजेक्शन का कोई खास फायदा नहीं है। इसलिए विश्व स्वास्थ्य संगठन ने रेमडेसिविर को मान्यता नहीं दी है। विशेषज्ञों का तर्क है कि इसकी एक्यूट रेस्पिेरटरी डिस्ट्रेस सिंड्रोम को रोकने में कोई अहम भूमिका नहीं है। वहीं रिसर्च में सामने आया कि रेमडेसिविर उस केस में दी जा सकती है जब ऑक्सीजन की हाई लेवल पर जरूरत होती है। इसे पहले सप्ताह में देने से राहत की अधिक संभावना बनती है।
डेक्सामेथासोन पर वैज्ञानिकों ने किया शोध
वैज्ञानिकों द्वारा करीब 2000 ऐसे कोरोना मरीजों पर शोध किया गया जिनका ऑक्सीजन लेवल कम था। उन मरीजों को डेक्सामेथासोन दवाई देने के 28 दिन बाद परिणाम सामने आए। जिसमें देखा गया कि मृत्यूदर पहले से कम थी।
लेकिन डेक्सामेथासोन के नुकसान भी अधिक है
डेक्सामेथासोन बहुत सस्ती दवा है। इसका उपयोग भारत में 1960 के दशक से किया जा रहा है। कई लोग जो बिना डिग्री के भी डॉ बन गए वह दर्द से राहत के लिए डेक्सामेथसोन ही देते हैं। विशेषज्ञों का कहना है कि इसके अधिक सेवन से इम्यूनिटी लेवल भी कम होने लगता है। नींद नहीं आने की समस्या, बेचैनी, वजन बढ़ना जैसी समस्या होने लगती है। डॉ और विशेषज्ञों का यह भी कहना है कि इस दवा को कभी भी डॉ की सलाह के बिना नहीं लें।
स्वास्थ्य मंत्री का ट्वीट
सेंट्रल हेल्थ मीनिस्टर द्वारा भी ट्वीट कर रेमडेसिविर इंजेक्शन के बारे में जनता को जागरूक किया गया। मंत्रालय द्वारा कहा गया किया कि, ‘रेमडेसिविर कोविड-19 में जीवन रक्षक दवा नहीं है। यह सिर्फ अस्पताल में प्रशासन की ओर से जारी की गई है। इसका उपयोग तब ही किया जाए जब हाई लेवल ऑक्सीजन की कमी होने लगे।’
